If the person making the Request for Disclosure, etc. is the individual concerned, a legal representative of a minor or adult ward, or an agent authorized by the person themselves to make the Request for Disclosure, etc., please attach the following documents and data (either (1) or (2)) in addition to the documents and data listed in 3 above.
